ABSTRACT

Purpose: Secondary acquired lacrimal duct obstruction (SALDO) is one of the complications of radioiodine therapy. SALDO is formed a few months after therapy if there is a sufficient uptake of radioactive iodine by the nasolacrimal duct. To date, risk factors leading to SALDO are unclear. The objective was to determine the correlation between the tear production level and radioactive iodine?131 uptake in the lacrimal ducts.Methods: Basal and reflex tear production was studied in 64 eyes prior to the therapy with radioactive iodine?131 after drug?induced hypothyroidism. The condition of the ocular surface was assessed using the Ocular Surface Disease Index (OSDI) questionnaire. Seventy?two hours after the radioactive iodine therapy, scintigraphy was performed, which determined the presence or absence of iodine?131 in the lacrimal ducts. T?statistics and the Mann–Whitney criterion were used to identify the differences between the groups. The differences were considered significant at P ? 0.05. The current tear production level in patients receiving radioiodine therapy was determined using a mathematical model. Results: A statistically significant difference between the basal (p = 0.044) and reflex (p = 0.015) tear production levels was found in cases with and without iodine?131 uptake by the lacrimal ducts. The probable current tear production level corresponds to the sum of basal and 10–20% of reflex tear production. The uptake of iodine?131 was found regardless of the OSDI results.Conclusion: The probability of iodine?131 uptake by the lacrimal ducts rises as the tear production level increases.

ABSTRACT

ABSTRACT Ocular metastases from systemic tumors are uncommon. The choroid is the most frequent target, with a preference for elderly individuals. Lung cancer is the predominant primary tumor that metastasizes to the eyes in males, although other ocular conditions such as uveitis and retinal lesions can mimic secondary tumor implants in ocular tissues. On fundoscopy, choroidal metastasis resembles other infectious processes, especially choroidal tuberculoma. Therefore, patients presenting with choroidal masses should undergo detailed clinical examinations, especially if the mass is the first manifestation of a systemic and severe disease. In this report, we describe a young man with a metastatic choroidal tumor secondary to papillary renal cell carcinoma mimicking a unilateral choroidal tuberculoma.


RESUMO A disseminação metastática ocular de tumores sistêmicos é incomum, ocorrendo principalmente na coroide e em pacientes idosos. O câncer de pulmão é considerado o principal tumor metastático ocular em homens, contudo, outras doenças oculares, como as uveítes e lesões retinianas, podem mimetizar os implantes secundários tumorais nos tecidos oculares. O aspecto fundoscópico das neoplasias da coroide pode apresentar similaridade com outros processos infecciosos, especialmente o tuberculoma de coroide. Dessa forma, a investigação clínica detalhada é de grande importância no diagnóstico de pacientes com massas coroideanas, especialmente quando configuram a primeira manifestação de uma doença sistêmica e grave. Relatamos um caso raro de metástase coroideana como primeira manifestação clínica do carcinoma de células renais em um homem jovem, mimetizando um tuberculoma de coroide.

RESUMEN: Gran parte de los pacientes con cáncer de colon (CC), son diagnosticados y tratados de forma electiva. Sin embargo, aproximadamente un 20 % de ellos debutará como una emergencia (obstrucción o perforación). El objetivo de este estudio fue determinar morbilidad postoperatoria (MPO) y supervivencia global (SVG) en pacientes resecados por CC perforado (CCP). Serie de casos retrospectiva de pacientes con CCP, sometidos a colectomía y linfadenectomía, de forma consecutiva, en Clínica RedSalud Mayor y Hospital de Temuco, Chile, entre 2010 y 2019. Las variables resultados fueron SVG y MPO. Otras variables de interés fueron: tiempo quirúrgico, resecabilidad, número de linfonodos resecados, estancia hospitalaria, mortalidad operatoria, recurrencia y supervivencia libre de enfermedad (SLE). Los pacientes fueron seguidos de forma clínica. Se utilizó estadística descriptiva, con medidas de tendencia central y dispersión; y análisis de SV con curvas de Kaplan Meier. Se intervinieron 15 pacientes (60 % mujeres), con una mediana de edad de 62 años. La localización más frecuente fue sigmoides (6 casos; 40,0 %). La resecabilidad de la serie fue 100 %. La medianas del tiempo quirúrgico, número de linfonodos resecados y estancia hospitalaria; fueron 80 min, 20 y 5 días respectivamente. La MPO fue 26,7 % (4 casos). Con una mediana de seguimiento de 36 meses, se verificó una recurrencia de 40,0 %. Por otra parte, la SVG y SLE a 5 años fue 46,7 % y 33,3 % respectivamente. Los resultados obtenidos, en términos de MPO y SVG, fueron similares a series internacionales.


SUMMARY: Most patients with colon cancer (CC) are diagnosed and treated electively. However, a fifth of them will debut as an emergency (obstruction or perforation). The aim of this study was to determine postoperative morbidity (POM) and overall survival (OS) in patients resected by perforated CC (PCC). Retrospective case series of patients with PCC undergoing colectomy and lymphadenectomy, consecutively, at RedSalud Mayor Clinic and Temuco hospital, Chile, between 2010 and 2019. The outcome variable were POM and OS. Other variables of interest were surgical time, resectability, number of resected lymph nodes, hospital stay, mortality, recurrence, and disease-free survival (DFS). Patients were followed clinically. Descriptive statistics was used (measures of central tendency and dispersion), and OS analysis was applying Kaplan Meier curves.15 patients (60 % women) were intervened, with a median age of 62 years. The most frequent location was the sigmoid colon (6 cases, 40.0 %). Resectability of the series was 100 %. Median surgical time, number of lymph nodes resected, and hospital stay; they were 80 min, 20 and 5 days respectively. POM was 26.7 % (4 cases). With a median follow-up of 36 months, recurrence was 40.0 %. On the other hand, OS and DFS at 5 years were 46.7 % and 33.3 %, respectively. The observed results, in terms of POM and OS, were like international series.

Resumo Fundamento A relação direta entre a doença arterial coronariana (DAC) e o câncer de pulmão não é bem conhecida. Objetivo Investigar a associação entre a gravidade anatômica da DAC e do câncer de pulmão. Métodos Trezentos pacientes, incluindo 75 recém-diagnosticados com câncer de pulmão e 225 pacientes correspondentes sem câncer, foram submetidos à angiografia coronária durante a internação, sem intervenção coronária percutânea (ICP) prévia nem enxerto de bypass da artéria coronária (CABG). O escore SYNTAX foi utilizado para avaliar a gravidade da DAC. Uma pontuação alta no escore foi definida como > 15 (o maior quartil do escore SYNTAX). O teste de tendência de Cochran-Armitage foi utilizado para verificar a distribuição dos escores dos pacientes. Uma análise de regressão logística foi utilizada para avaliar a associação entre a gravidade da DAC e o câncer de pulmão. Os valores de p foram estabelecidos quando o nível de significância era 5%. Resultados A tendência de distribuição dos escores SYNTAX dos pacientes por quartis foi diferente entre aqueles com câncer de pulmão e controles (do quartil mais baixo ao mais alto: 20,0%; 20,0%; 24,0%; 36,0% vs. 26,7%; 26,2%; 25,8%; 21,3%; p=0,022). A pontuação no escore SYNTAX foi mais alta em pacientes com câncer do que nos pacientes controle (36,0% vs. 21,3%, p=0,011).O maior quartil do escore demonstrou mais riscos de desenvolver câncer de pulmão em comparação ao quartil mais baixo (OR: 2.250, IC95%: 1.077 a 4.699 ; P -trend= 0,016). Após ajustes, os pacientes no maior quartil do escore SYNTAX tinham mais risco de desenvolver câncer de pulmão (OR: 2.1o49, IC95%: 1.008 a 4.584; P -trend= 0,028). Pacientes com escores SYNTAX alto (> 15) tinham 1.985 mais chances de ter câncer de pulmão (IC95%: 1.105-3.563, P= 0,022). Conclusão A gravidade anatômica da DAC está associada ao risco de câncer de pulmão, o que indica que um rastreamento completo deste tipo de câncer possa ser mais significativo entre pacientes com DAC.


Abstract Background The direct relationship between coronary artery disease (CAD) and lung cancer is not well known. Objective To investigate the association between the anatomical severity of CAD and lung cancer. Methods Three-hundred study patients, including 75 recently diagnosed lung cancer patients and 225 matched non-cancer patients, underwent coronary angiography during hospitalization without previous percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG). The SYNTAX score (SXscore) was used to assess the severity of CAD. A high SXscore (SXhigh) grade was defined as SXscore > 15 (the highest quartile of the SXscore). The Cochran-Armitage test for trend was used to assess the distribution of patients' SXscores. Logistic regression analysis was used to assess the association between the severity of CAD and lung cancer. P-values were set when significance level was 5%. Results The distribution trend of patients' SXscore by quartiles was different between lung cancer patients and control patients (from the lowest to the highest quartile: 20.0%, 20.0%, 24.0%, 36.0% vs. 26.7%, 26.2%, 25.8%, 21.3%, p=0.022). The SX high rate was higher in lung cancer patients than in control patients (36.0% vs. 21.3%, p=0.011).The highest quartile of the SXscore showed higher risk of lung cancer in comparison to the lowest quartile (OR: 2,250, 95%CI: 1,077 to 4,699 ; P-trend= 0.016). After adjustment, patients in the highest quartile of the SXscore had higher risk of lung cancer (OR: 2,149, 95%CI: 1,008 to 4,584; P-trend= 0.028). Patients with high SXscore (> 15) had 1,985 times more chances of having lung cancer (95%CI: 1,105-3,563, P= 0.022). Conclusions The anatomical severity of CAD is associated with the risk of lung cancer, which indicates that a thorough lung cancer screening may be significant among severe CAD patients.

RESUMEN Fundamento: la incidencia de cáncer de pulmón en la provincia Sancti Spíritus es alta y su diagnóstico por lo general, se realiza en etapas avanzadas por lo cual el tratamiento quirúrgico no siempre es posible. Objetivo: analizar las principales características y resultados de los pacientes operados por cáncer de pulmón. Métodos: se realizó un estudio observacional descriptivo de corte transversal en el servicio de Cirugía General del Hospital Universitario General Camilo Cienfuegos en la provincia Sancti Spiritus en el período comprendido entre enero de 1996 y diciembre de 2019. El universo consistió en 83 pacientes operados con cáncer de pulmón que se sometieron a resección acompañados de muestreo de ganglios linfáticos mediastinales. Resultados: el sexo masculino predominó en el grupo estudiado. Los grupos de edades más frecuentes para ambos sexos fueron los de 51 a 60 años y los mayores de 60 años. El hábito de fumar estuvo presente en la mayoría de los pacientes. La localización tumoral más predominó fue en los lóbulos superiores y la lobectomía fue la técnica más realizada. Las etapas patológicas que predominaron fueron la lB y llB, seguidas de la lllA. El carcinoma de células escamosas fue el tipo histológico más frecuente. Las atelectasias y las infecciones respiratorias fueron las complicaciones más frecuentes. Los fallecidos fueron pocos. Conclusiones: las características demográficas de los pacientes estudiados se corresponden a otras series de pacientes con cáncer del pulmón. La localización tumoral en los lóbulos superiores, el carcinoma epidermoide, así como las etapas tempranas, predominaron en la serie estudiada. La lobectomía fue la técnica quirúrgica que se utilizó con mayor frecuencia y dentro de las complicaciones, las respiratorias fueron las más frecuentes.


ABSTRACT Background: the incidence of lung cancer in the province of Sancti Spíritus is high and its diagnosis is usually made in advanced stages, for which surgical treatment is not always possible. Objective: to analyze the main characteristics and results of patients operated on for lung cancer. Methods: a cross-sectional descriptive observational study was carried out in the General Surgery service of the Camilo Cienfuegos General University Hospital in the Sancti Spiritus province in the period between January 1996 and December 2019. The universe consisted of 83 patients operated with lung cancer who underwent resection accompanied by mediastinal lymph node sampling. Results: the male sex predominated in the studied group. The most frequent age groups for both sexes were those from 51 to 60 years old and those over 60 years old. Smoking was present in most of the patients. The most frequent tumor location was in the upper lobes and lobectomy was the most performed technique. The pathological stages that predominated were lB and llB, followed by lllA. Squamous cell carcinoma was the most frequent histological type. Atelectasis and respiratory infections were the most frequent complications. The deceased were few. Conclusions: the demographic characteristics of the patients studied correspond to other series of patients with lung cancer. Tumor location in the upper lobes, squamous cell carcinoma, as well as early stages, predominated in the series studied. Lobectomy was the most frequently used surgical technique and among complications, respiratory ones were the most frequent.

RESUMEN Fundamento: el estudio ganglionar del mediastino en los pacientes afectos de cáncer pulmonar es de vital importancia, al permitir trazar la estrategia terapéutica. Objetivo: evaluar la técnica de linfoadenectomía transcervical como parte de la sistemática diagnóstica en el cáncer pulmonar. Métodos: se realizó una investigación analítica, longitudinal prospectiva en pacientes, se usó la linfoadenectomía transcervical como parte de la sistemática diagnóstica del cáncer pulmonar en el Hospital Universitario Manuel Ascunce Domenech, de la provincia Camagüey. El universo estuvo compuesto por 346 pacientes y la muestra, de tipo probabilístico aleatorio simple, quedó conformada por 65 pacientes. Resultados: predominaron los pacientes masculinos y mayores de 60 años. El tipo histológico más frecuente fue el adenocarcinoma y los lóbulos superiores fueron los más afectados. Se extirparon un total de 205 ganglios y la mayoría fueron positivos, la negatividad, en la histología, predominó en los ganglios menores de 1 cm diagnosticados en el estudio tomográfico. Existió migración de estadios en la correlación clínico patológica, así como la sensibilidad y especificidad fueron de un 100 % en la muestra estudiada. Las complicaciones fueron escasas. Conclusiones: la linfoadenectomía transcervical constituye una herramienta diagnóstica en los pacientes con cáncer pulmonar y afectación ganglionar mediastinal.


ABSTRACT Background: the lymph node study of the mediastinum in patients with lung cancer is of vital importance, as it allows the therapeutic strategy to be outlined. Objective: to evaluate the transcervical lymphadenectomy technique as part of the diagnostic system in lung cancer. Methods: a prospective longitudinal analytical investigation was carried out in patients in whom transcervical lymphadenectomy was used as part of the diagnostic system for lung cancer at the Hospital Universitario Manuel Ascunce Domenech, in the province of Camagüey. The universe was made up of 346 patients and the sample, of a simple random probability type, consisted of 65 patients. Results: male patients over 60 years prevailed. The most frequent histological type was adenocarcinoma and the upper lobes were the most affected. A total of 205 nodes were removed and the majority were positive, negativity, in histology, predominated in nodes less than 1 cm diagnosed in the tomographic study. There was migration of stages in the clinical pathological correlation, as well as the sensitivity and specificity were 100% in the studied sample. Complications were rare. Conclusions: transcervical lymphadenectomy constitutes a diagnostic tool in patients with lung cancer and mediastinal lymph node involvement.

ABSTRACT

Resumo O desafio imposto ao sistema de saúde pela pandemia da COVID-19 faz com que haja uma necessidade de readequações de rotinas e serviços de saúde, com os objetivos de controlar a disseminação do vírus e preservar a saúde. Torna-se ainda mais importante o manejo seguro e correto dos pacientes dos grupos de risco, como os pacientes idosos, os portadores de doenças cardiovasculares e os pacientes com câncer. Dessa forma, a cardio-oncologia ganha novo dimensionamento, no intuito de se adequar às necessidades dos pacientes diante de uma pandemia, reestruturando o sistema de atendimento de forma a oferecer qualidade e segurança na assistência à saúde.


Abstract The challenges that the COVID-19 pandemic cretead to the healthcare system have made it necessary to adapt routines and services, with the objectives of controlling the spread of the virus and preserving health. Safe and correct management of patients in risks groups, such as elderly patients, patients with cardiovascular diseases, and patients with cancer, has become even more important. Thus, cardio-oncology has gained a new dimension, with the aim of adapting to patients' needs during the pandemic by restructuring the system of care in a manner that offers quality and safety in healthcare.

Resumo Fundamento A doença arterial coronariana (DAC) associada à quimioterapia está se tornando um tema emergente na prática clínica. Contudo, o mecanismo subjacente da quimioterapia associada à DAC permanence incerto. Objetivos O estudo investigou a associação entre a quimioterapia e as anomalias anatômicas ateroscleróticas das artérias coronárias dentre pacientes com cancer de pulmão. Métodos Foram incluídos pacientes submetidos à angiografia coronária (AGC), entre 2010 e 2017, com câncer de pulmão prévio. Os fatores de risco associados à DAC e os dados sobre o câncer de pulmão foram avaliados. Avaliamos as anomalias das artérias coronárias de acordo com o escore SYNTAX (SXescore) calculado à AGC. Na análise de regressão logística, o escore SYNTAX foi classificado como alto (SXescoreALTO) se ≥22. Os dados foram analisados através de estatística descritiva e análise de regressão. Resultados Ao todo, 94 pacientes foram incluídos no estudo. O SXescore foi mais alto no grupo com quimioterapia quando comparado com o grupo sem quimioterapia (25,25, IIQ [4,50-30,00] versus 16,50, IIQ [5,00-22,00]; p = 0,0195). A taxa do SXescoreALTO foi maior no grupo com quimioterapia do que no no grupo sem quimioterapia (58,33% versus 25,86; p = 0,0016). Tanto a análise de regressão logística univariada (OR: 4,013; 95% IC:1,655-9,731) quanto a multivariada (OR: 5,868; 95% IC:1,778-19,367) revelaram que a quimioterapia aumentou o risco de uma maior taxa do SXescoreALTO. A análise multivariada de regressão logística Stepwise mostrou que o risco para DAC anatômica mais grave aumenta com a quimioterapia como um todo em 5.323 vezes (95% IC: 2,002-14,152), e com o regime à base de platina em 5,850 vezes (95% IC: 2,027-16,879). Conclusões A quimioterapia está associada com a complexidade e gravidade anatômica da DAC, o que pode explicar, em parte, o maior risco de DAC associada à quimioterapia dentre pacientes com câncer de pulmão. Abstract Background Chemotherapy-related coronary artery disease (CAD) is becoming an emerging issue in clinic. However, the underlying mechanism of chemotherapy-related CAD remains unclear. Objective The study investigated the association between chemotherapy and atherosclerotic anatomical abnormalities of coronary arteries among lung cancer patients. Methods Patients undergoing coronary angiography (CAG) between 2010 and 2017, who previously had lung cancer, were examined. Risk factors associated with CAD and information about lung cancer were evaluated. We assessed coronary-artery abnormalities by SYNTAX score (SXscore) based on CAG. In logistic-regression analysis, we defined high SXscore (SXhigh) grade as positive if ≥22. Data were analyzed through descriptive statistics and regression analysis. Results A total of 94 patients were included in the study. The SXscore was higher in the chemotherapy group than in the non-chemotherapy group (25.25, IQR [4.50-30.00] vs. 16.50, IQR [ 5.00-22.00], p = 0.0195). The SXhigh rate was greater in the chemotherapy group than in the non-chemotherapy group (58.33% vs. 25.86; p = 0.0016). Both univariate (OR:4.013; 95% CI:1.655-9.731) and multivariate (OR:5.868; 95% CI:1.778-19.367) logistic-regression analysis revealed that chemotherapy increased the risk of greater SXhigh rates. Multivariate stepwise logistic-regression analysis showed the risk of more severe anatomical CAD is increased by chemotherapy as a whole by 5.323 times (95% CI: 2.002-14.152), and by platinum-based regimens by 5.850 times (95% CI: 2.027-16.879). Conclusions Chemotherapy is associated with anatomical complexity and severity of CAD, which might partly account for the higher risk of chemotherapy-related CAD among lung cancer patients. RESUMO Objetivo: Descrever a ocorrência de delirium em pacientes com câncer internados em unidade de terapia intensiva, segundo características clínicas e demográficas. Métodos: Estudo retrospectivo realizado com 135 adultos internados na unidade de terapia intensiva de um hospital público especializado em oncologia, localizado na cidade do Rio de Janeiro, entre os meses de janeiro e março de 2016. Foram utilizados o teste exato de Fischer e o teste de associação linear para identificar diferenças estatisticamente significativas na ocorrência de delirium entre, respectivamente, variáveis categóricas e ordinais, considerando valor de p < 0,05. Resultados: A ocorrência global de delirium foi igual a 39,3%, sendo foi mais frequente entre pessoas com idade igual ou superior a 60 anos, com performance status que requeria grande assistência/acamado, internaram na unidade de terapia intensiva por problemas clínicos, em uso de drogas sedativas, submetidas à quimioterapia, e permaneceram 8 ou mais dias internados na unidade de terapia intensiva. Considerando apenas os pacientes sob ventilação mecânica, a ocorrência global de delirium foi de 64,6%, e apenas o tempo de permanência na unidade de terapia intensiva ≥ 8 dias mostrou diferença estatisticamente significativa. Conclusão: A ocorrência de delirium em pacientes críticos com câncer é elevada. Considerando apenas aqueles sob ventilação mecânica, a ocorrência desse fenômeno é ainda maior.


ABSTRACT Objective: To describe the occurrence of delirium in cancer patients admitted to the intensive care unit according to clinical and demographic characteristics. Methods: A retrospective study was conducted with 135 adults admitted to the intensive care unit of a public cancer hospital in the city of Rio de Janeiro, Brazil, between January and March 2016. Fisher's exact test and the linear association test were used to identify statistically significant associations between the occurrence of delirium and categorical and ordinal variables, respectively, considering a p-value < 0.05. Results: The overall occurrence of delirium was 39.3%. Delirium was more frequent among individuals aged 60 years or older and those who required extensive assistance or were bedbound, were admitted to the intensive care unit for clinical reasons, were using sedative drugs, were undergoing chemotherapy, and those who remained 8 or more days in the intensive care unit. Considering only patients on mechanical ventilation, the overall occurrence of delirium was 64.6%, and only a length of stay in the intensive care unit ≥ 8 days showed a statistically significant association with delirium. Conclusion: The occurrence of delirium in critically ill cancer patients is high. When only those on mechanical ventilation are considered, the occurrence of delirium is even greater.

ABSTRACT Objective: To describe our experience with video-assisted thoracic surgery (VATS) for anatomic pulmonary resection at a referral center for thoracic surgery in Brazil. Methods: All patients who underwent anatomic pulmonary resection by VATS between 2010 and 2015 were included. Clinical and pathological data, as well as postoperative complications, were analyzed. Results: A total of 117 pulmonary resections by VATS were performed, of which 98 were lobectomies and 19 were anatomic segmentectomies. The mean age of the patients was 63.6 years (range, 15-86 years). Females predominated (n = 69; 59%). The mean time to chest tube removal was 2.47 days, and the mean length of ICU stay was 1.88 days. The mean length of hospital stay was 4.48 days. Bleeding ≥ 400 mL occurred in 15 patients. Conversion to thoracotomy was required in 4 patients. Conclusions: Our results are similar to those published in major international studies, indicating that VATS is an important strategy for pulmonary resection. They also show that VATS can be safely performed with adequate training. This technique should be used more often for the treatment of lung diseases in Brazil.


RESUMO Objetivo: Relatar a experiência com cirurgia torácica videoassistida (CTVA) para ressecções pulmonares anatômicas em um centro nacional de referência de cirurgia torácica no Brasil. Métodos: Foram incluídos todos os pacientes tratados com ressecções pulmonares anatômicas por CTVA entre 2010 e 2015 e analisados dados clínicos e patológicos, assim como complicações pós-operatórias. Resultados: Foram realizadas 117 ressecções pulmonares por CTVA, sendo 98 lobectomias e 19 segmentectomias anatômicas. A média de idade foi de 63,6 anos (variação, 15-86 anos), sendo a maioria mulheres (n = 69; 59%). A média de tempo de permanência com dreno foi de 2,47 dias e a de estada em UTI foi de 1,88 dias. A média de tempo de internação foi de 4,48 dias. Sangramento ≥ 400 ml ocorreu em 15 pacientes. Houve conversão para toracotomia em 4 pacientes. Conclusões: Nossos resultados vão ao encontro de publicações em grandes séries internacionais, corroborando que a CTVA é uma importante via de execução de ressecções pulmonares e que pode ser realizada com segurança quando há treinamento adequado, devendo ser mais utilizada no Brasil para o tratamento das doenças pulmonares.

Resumo Objetivo: Comparar a qualidade do sono, a depressão e a esperança em mulheres com câncer de mama ao longo de aproximadamente um ano. Métodos: Estudo longitudinal, com 107 mulheres, em hospital universitário. Foram utilizados os instrumentos: questionário de caracterização sociodemográfica e clínica; Índice de Qualidade do Sono de Pittsburgh; Inventário de Depressão de Beck e Escala de Esperança de Herth. Os dados foram coletados em T0, antes da cirurgia de retirada do tumor, e após a mesma em: T1, em média 3,2 meses; T2, em média 6,1 meses; em T3, em média 12,4 meses. Resultados: A má qualidade do sono persistiu ao longo do seguimento; a esperança aumentou em T1; a proporção de mulheres com depressão moderada e grave elevou-se em T3. Houve correlação significativa entre os escores indicativos de qualidade do sono, de depressão e de esperança nos quatro tempos. Conclusão: Os achados deste estudo indicam a importância do seguimento por tempo prolongado.


Abstract Objectives: To compare sleep quality, depression, and hope in women with breast cancer over an approximate period of 1 year. Methods: This longitudinal study included 107 women assisted at a teaching hospital. The following instruments were used: Questionnaire on Socio-demographic and Clinical Characteristics; Pittsburgh Sleep Quality Index; Beck Depression Inventory; and Herth Hope Scale. Data were collected at T0, before tumor removal surgery, and after surgery at T1 (average of 3.2 months), T2 (average of 6.1 months), and T3 (average of 12.4 months). Results: Poor sleep quality persisted over the follow-up; hope increased at T1; the proportion of women with moderate and severe depression increased at T3. There was a significant correlation among scores indicating sleep quality, depression, and hope in all four time points. Conclusion: Our study findings suggest the significance of long-term follow-up.

ABSTRACT Objective: this study assessed the influence of pre-operative body mass index (BMI) has upon lymphedema, scar tissue adhesion, pain, and heaviness in the upper limb at two years after surgery for breast cancer. Methods: retrospective analysis of 631 medical records of women who underwent surgery for breast cancer and were referred to the Physiotherapy Program at Prof. Dr. José Aristodemo Pinotti Women's Hospital of the Center for Integral Women's Health Care, CAISM/UNICAMP between January 2006 and December 2007. Results: mean age of women was 56.5 years (±13.7 years) and the most part (55%) were overweight or obese, surgical stages II and III were present in 63% of women studied. Radical mastectomy was the most frequent surgery (54.4%), followed by quadrantectomy (32.1%). In the first year after surgery, there was no significant association between BMI categories and incidence of scar tissue adhesion, pain, heaviness and lymphedema. In the second year, overweight and obese women had higher rates of heaviness in the upper limb and lymphedema. For lymphedema, there was a significant difference among BMI categories (p=0.0268). Obese women are 3.6 times more likely to develop lymphedema in the second year after surgery (odds ratio 3.61 95% CI 1.36 to 9.41). Conclusion: BMI ≥25kg/m2 prior to treatment for breast cancer can be considered a risk factor for developing lymphedema in the two years after surgery. There was no association between BMI and the development of other complications.


RESUMO Avaliou-se a influência do índice de massa corporal (IMC) pré-operatória na ocorrência de linfedema, aderência cicatricial, dor e peso no membro superior nos primeiros dois anos após cirurgia para câncer de mama. O estudo é uma análise retrospectiva, secundária de 631 prontuários de mulheres submetidas à cirurgia para câncer de mama e encaminhadas ao Programa de Fisioterapia do Hospital Professor Dr. José Aristodemo Pinotti do Centro de Atenção Integral à Saúde da Mulher, CAISM /UNICAMP, entre janeiro de 2006 e dezembro de 2007. Eram mulheres com idade média de 56,5 anos (±13,7 anos), a maioria (55%) com sobrepeso ou obesa. Os estádios clínicos II e III foram encontrados em 63% das mulheres. Mastectomia radical foi a cirurgia mais frequente (54,4%), seguida por quadrantectomia (32,1%). No primeiro ano após a cirurgia não houve associação significativa entre as categorias do índice de massa corporal e incidência de aderência cicatricial, dor, peso e linfedema. No segundo ano, mulheres com sobrepeso e obesidade apresentaram maiores taxas de peso no membro superior e linfedema. Para linfedema houve diferença significativa entre as categorias de índice de massa corporal (p=0,0268). Mulheres obesas têm 3,6 vezes mais chance de desenvolver linfedema no segundo ano após a cirurgia (odds ratio 3,61 95% IC 1,36-9,41). Concluiu-se que IMC ≥25kg/m2 anterior ao tratamento para câncer de mama pode ser considerado fator de risco para desenvolvimento do linfedema dois anos após a cirurgia. Não houve associação entre IMC e outras complicações.

Método: análisis retrospectivo de 631 historias clínicas de mujeres sometidas a cirugía para el cáncer de mama y encaminadas al Programa de Fisioterapia do Centro de Atención Integral de la Salud de la Mujer - Professor Dr. José Aristodemo Pinotti - CAISM /UNICAMP entre enero de 2006 y diciembre de 2007. Resultados: mujeres de mediana edad 56,5 años (±13,7 años), 55% tenían sobrepeso u obesidad. Etapas II y III del cáncer se encontraron en el 63% de las mujeres. Cirugía de mastectomía radical fue la más frecuente (54,4%), seguido de cuadrantectomía (32,1%). En el primer año después de la cirugía no hubo asociación significativa entre categorías de índice de masa corporal y incidencia de la adhesión del tejido de cicatriz, dolor, peso y linfedema. En el segundo año, sobrepeso y obesidad tenían mayores tasas de peso y linfedema. Para linfedema hubo diferencias significativas entre categorías de índice de masa corporal (p=0,0268). Las mujeres obesas tienen 3,6 veces más probabilidades de desarrollar linfedema en el segundo año después de la cirugía (odds ratio 3,61 IC del 95%: 1,36 a 9,41). La conclusión és índice de masa corporal ≥25kg/m2 antes del tratamiento para el cáncer de mama puede ser considerado un factor de riesgo para desarrollo de linfedema, dos años después de la cirugía. No hubo asociación entre el índice de masa corporal y otras complicaciones.

RESUMEN Objetivo: Determinar la influencia de las complicaciones postoperatorias sobre los resultados clínicos en pacientes operados por cáncer torácico y gastrointestinal. Métodos: Se realizó un estudio de cohorte prospectivo de 179 pacientes consecutivos que fueron operados de tórax o vías digestivas por cáncer y admitidos en una unidad de cuidados intensivos oncológicos. Se evaluó la incidencia de las complicaciones postoperatorias mediante el Postoperative Morbidity Survey y su influencia sobre la mortalidad y estadía hospitalaria. Resultados: Se presentaron complicaciones postoperatorias en 54 sujetos (30,2%); las más frecuentes fueron las pulmonares (14,5%), el dolor (12,9%), las cardiovasculares (11,7%), las infecciosas (11,2%) y las de la herida quirúrgica (10,1%). En el análisis multivariado de regresión logística las complicaciones pulmonares (OR 18,68; IC95% 5,59 - 62,39; p < 0,0001), cardiovasculares (OR 5,06; IC95% 1,49 - 17,13; p = 0,009), gastrointestinales (OR 26,09; IC95% 6,80 - 100,16; p < 0,0001), infecciosas (OR 20,55; IC95% 5,99 - 70,56; p < 0,0001) y renales (OR 18,27; IC95% 3,88 - 83,35; p < 0,0001) se relacionaron de forma independiente con la mortalidad hospitalaria. La ocurrencia de al menos una complicación incrementó la probabilidad de permanecer hospitalizado (Log Rank test; p = 0,002). Conclusiones: Las complicaciones postoperatorias son trastornos frecuentes y asociados con malos resultados clínicos, por lo que se deben realizar cambios estructurales y de proceso para reducir la morbilidad y mortalidad postoperatorias.


ABSTRACT Objective: This study sought to determine the influence of postoperative complications on the clinical outcomes of patients who underwent thoracic and gastrointestinal cancer surgery. Methods: A prospective cohort study was conducted regarding 179 consecutive patients who received thorax or digestive tract surgery due to cancer and were admitted to an oncological intensive care unit. The Postoperative Morbidity Survey was used to evaluate the incidence of postoperative complications. The influence of postoperative complications on both mortality and length of hospital stay were also assessed. Results: Postoperative complications were found for 54 patients (30.2%); the most common complications were respiratory problems (14.5%), pain (12.9%), cardiovascular problems (11.7%), infectious disease (11.2%), and surgical wounds (10.1%). A multivariate logistic regression found that respiratory complications (OR = 18.68; 95%CI = 5.59 - 62.39; p < 0.0001), cardiovascular problems (OR = 5.06, 95%CI = 1.49 - 17.13; p = 0.009), gastrointestinal problems (OR = 26.09; 95%CI = 6.80 - 100.16; p < 0.0001), infectious diseases (OR = 20.55; 95%CI = 5.99 - 70.56; p < 0.0001) and renal complications (OR = 18.27; 95%CI = 3.88 - 83.35; p < 0.0001) were independently associated with hospital mortality. The occurrence of at least one complication increased the likelihood of remaining hospitalized (log-rank test, p = 0.002). Conclusions: Postoperative complications are frequent disorders that are associated with poor clinical outcomes; thus, structural and procedural changes should be implemented to reduce postoperative morbidity and mortality.

ABSTRACT

Objective: to discuss the participation of Plastic Surgery in the reconstruction of the chest wall, highlighting relevant aspects of interdisciplinaryness. Methods: we analyzed charts from 20 patients who underwent extensive resection of the thoracic integument, between 2000 and 2014, recording the indication of resection, the extent and depth of the raw areas, types of reconstructions performed and complications. Results: among the 20 patients, averaging 55 years old, five were males and 15 females. They resections were: one squamous cell carcinoma, two basal cell carcinomas, five chondrosarcomas and 12 breast tumors. The extent of the bloody areas ranged from 4x9 cm to 25x40 cm. In 12 patients the resection included the muscular plane. In the remaining eight, the tumor removal achieved a total wall thickness. For reconstruction we used: one muscular flap associated with skin grafting, nine flaps and ten regional fasciocutaneous flaps. Two patients undergoing reconstruction with fasciocutaneous flaps had partially suffering of the flap, solved with employment of a myocutaneous flap. The other patients displayed no complications with the techniques used, requiring only one surgery. Conclusion: the proper assessment of local tissues and flaps available for reconstruction, in addition to the successful integration of Plastic Surgery with the specialties involved in the treatment, enable extensive resections of the chest wall and reconstructions that provide patient recovery.


Objetivo: discorrer sobre a participação da Cirurgia Plástica na reconstrução da parede torácica, ressaltando os aspectos relevantes das associações interdisciplinares. Métodos: foram analisados prontuários de 20 pacientes submetidos a extensas ressecções do tegumento torácico, no período entre 2000 e 2014, quanto à indicação das ressecções, à extensão e à profundidade das áreas cruentas, aos tipos de reconstruções realizadas e às complicações. Resultados: entre os 20 pacientes, com média de 55 anos de idade, cinco eram do sexo masculino e 15 do feminino. Foram ressecados: um carcinoma espinocelular, dois carcinomas basocelulares, cinco condrossarcomas e 12 tumores de mama. A extensão das áreas cruentas variou de 4x9 cm até 25x40 cm. Em 12 pacientes as ressecções abrangeram o plano muscular. Nos oito restantes, a retirada do tumor atingiu a espessura total da parede. Para reconstrução foram utilizados: um retalho muscular associado à enxertia de pele, nove retalhos miocutâneos e dez retalhos fasciocutâneos da região. Em dois pacientes submetidos à reconstrução com retalhos fasciocutâneos houve sofrimento parcial do retalho, resolvido com o emprego de retalho miocutâneo. Nos outros pacientes não houve intercorrências com as técnicas empregadas, sendo necessária somente uma cirurgia. Conclusão: a adequada avaliação dos tecidos locais e dos retalhos disponíveis para a reconstrução, além da boa integração da Cirurgia Plástica com as especialidades envolvidas no tratamento, possibilitam extensas ressecções da parede torácica e reconstruções que propiciam a recuperação do paciente.

RESUMOObjetivo:Revisar sistematicamente os principais métodos para avaliação do risco nutricional utilizados em pacientes oncológicos graves e apresentar aqueles que melhor avaliam os riscos e preveem desfechos clínicos relevantes neste grupo de pacientes, além de discutir as vantagens e as desvantagens destes métodos, segundo a literatura atual.Métodos:O estudo consistiu de uma revisão sistemática com base na análise de artigos obtidos nas bases de dados PubMed, LILACS e SciELO, realizando as buscas com os termos em inglês: “nutritional risk assessment”, “critically ill” e “cancer”.Resultados:Apenas 6 (17,7%) dos 34 artigos inicialmente obtidos cumpriam os critérios para inclusão e foram selecionados para revisão. Os principais desfechos destes estudos foram que o gasto de energia em repouso se associou com subnutrição e superalimentação. O escore elevado da Avaliação Subjetiva Global - Produzida pelo Paciente associou- se de forma significante com baixa ingestão de alimentos, perda de peso e desnutrição. Em termos de marcadores bioquímicos, níveis mais elevados de creatinina, albumina e ureia se associaram de forma significante com mortalidade mais baixa. Os piores índices de sobrevivência foram encontrados para pacientes com condições de desempenho piores, conforme avaliação usando o Eastern Cooperative Oncologic Group performance status, escore prognóstico de Glasgow elevado, baixa albumina/hipoalbuminemia, elevado escore da Avaliação Subjetiva Global - Produzida Pelo Paciente e para níveis elevados de fosfatase alcalina. Valores de avaliação do Índice de Risco Nutricional Geriátrico inferiores a 87 se associaram de forma significante com mortalidade. O escore pelo índice prognóstico inflamatório nutricional se associou com condição nutricional anormal em pacientes oncológicos graves. Dentre os estudos revisados que avaliaram apenas peso e índice de massa corporal, não se encontrou qualquer desfecho clínico significante.Conclusão:Nenhum dos métodos revisados ajudou a definir o risco entre esses pacientes. Portanto, sugere-se a avaliação por meio da quantificação da perda de peso e dos níveis séricos, preferivelmente em combinação com outros métodos utilizando escores como o Eastern Cooperative Oncologic Group performance status, o escore prognóstico de Glasgow e a Avaliação Subjetiva Global - Produzida Pelo Paciente, já que seu uso é simples, factível e útil em tais casos.


ABSTRACTObjective:To systematically review the main methods for nutritional risk assessment used in critically ill cancer patients and present the methods that better assess risks and predict relevant clinical outcomes in this group of patients, as well as to discuss the pros and cons of these methods according to the current literature.Methods:The study consisted of a systematic review based on analysis of manuscripts retrieved from the PubMed, LILACS and SciELO databases by searching for the key words “nutritional risk assessment”, “critically ill” and “cancer”.Results:Only 6 (17.7%) of 34 initially retrieved papers met the inclusion criteria and were selected for the review. The main outcomes of these studies were that resting energy expenditure was associated with undernourishment and overfeeding. The high Patient-Generated Subjective Global Assessment score was significantly associated with low food intake, weight loss and malnutrition. In terms of biochemical markers, higher levels of creatinine, albumin and urea were significantly associated with lower mortality. The worst survival was found for patients with worse Eastern Cooperative Oncologic Group - performance status, high Glasgow Prognostic Score, low albumin, high Patient-Generated Subjective Global Assessment score and high alkaline phosphatase levels. Geriatric Nutritional Risk Index values < 87 were significantly associated with mortality. A high Prognostic Inflammatory and Nutritional Index score was associated with abnormal nutritional status in critically ill cancer patients. Among the reviewed studies that examined weight and body mass index alone, no significant clinical outcome was found.Conclusion:None of the methods reviewed helped to define risk among these patients. Therefore, assessment by a combination of weight loss and serum measurements, preferably in combination with other methods using scores such as Eastern Cooperative Oncologic Group - performance status, Glasgow Prognostic Score and Patient-Generated Subjective Global Assessment, is suggested given that their use is simple, feasible and useful in such cases.

OBJETIVO: Estudar a frequência de neoplasias em uma amostra da população brasileira feminina com lúpus eritematoso sistêmico. MÉTODOS: Este é um trabalho retrospectivo de revisão de prontuários de 395 mulheres com lúpus eritematoso sistêmico, diagnosticadas pela presença de, pelo menos, 4 dos critérios classificatórios do Colégio Americano de Reumatologia para diagnóstico dessa doença e acompanhadas nos últimos 10 anos em um ambulatório especializado em Reumatologia. Foram coletados dados demográficos (idade e etnia das pacientes), dados acerca de duração de doença, de uso dos principais imunomoduladores e de diagnóstico de neoplasias. As informações obtidas foram tabuladas em tabelas de frequência e de contingência. A taxa de incidência de neoplasias nas mulheres com lúpus foi comparada com a da população em geral para a mesma região demográfica, nos últimos dez anos, utilizando, para isso, dados publicados pelo Instituto Nacional de Câncer (INCA). A associação dos dados foi feita pelos testes de Fisher e do χ2, quando os dados eram nominais, e de Mann-Whitney, quando numéricos. Foi adotada a significância de 5%. RESULTADOS: Foram identificados 22 casos de tumores malignos (22 casos/395 ou 5,5% da amostra) sendo os mais comuns os de útero (10 casos/395 ou 2,5% da amostra) e de mama (9 casos/395 ou 2,2% da amostra). A presença de tumores estava associada com tempo de duração de doença (p=0,006), não sendo observada influência de tratamento com metotrexate (p=0,1), azatioprina (p=0,9), ciclofosfamida (p=0,6) e glicocorticoides (p=0,3). PURPOSE: To study the incidence of tumors in a Brazilian sample of women with systemic lupus erythematosus. METHODS: This is a retrospective study of 395 medical charts from women with systemic lupus erythematosus diagnosed by the presence of at least 4 of the American College of Rheumatology classification criteria for the diagnosis of this disease and followed for the last 10 years in a rheumatology outpatient clinic. Demographic data (age and ethnicity of patients), data on disease duration, use of immunomodulators and on the presence of neoplasms were listed. Results are presented in frequency and contingency tables. The incidence rate of malignancies in women with lupus was compared with that of the general population for the same demographic region for the past ten years, using data published by the Brazilian National Cancer Institute (INCA). Association studies were carried out by the Fisher and χ2 tests, when the data were nominal, and by Mann-Whitney test, when numeric. The level of significance was set at 5%. RESULTS: Twenty-two cases of malignant tumors were identified during these 10 years of follow-up (22 cases/395 or 5.5% of the sample), being the most common those of the uterine cervix (10 cases/395 or 2.5% of the sample) and breast cancer (9 cases/395 or 2.2% of the sample). The presence of tumors was associated with disease duration (p=0.006) and was not influenced by treatment with methotrexate (p=0.1), azathioprine (p=0.9), cyclophosphamide (p=0.6) and glucocorticoids (p=0.3). Breast and uterine cervix tumors were more common in systemic lupus erythematosus women than in the general population (p<0.0001 for both). CONCLUSION: A high prevalence of malignant tumors was found in this sample, with tumors being more common in patients with longer disease duration. ABSTRACT

O macroadenoma de hipófise é um tumor de evolução lenta e que apresenta importantes distúrbios visuais como baixa acuidade visual e alterações campimétricas. Estes sintomas levam os pacientes a procurarem, muitas vezes, os oftalmologistas. Neste caso, analisaremos uma paciente portadora de glaucoma que apresentava, associadamente, um macroadenoma hipofisário.


The pituitary adenoma is a tumor of slow evolution and has important visual disturbances such as low visual acuity and field defects. These symptoms lead patients to seek often ophthalmologists. In this case, we will analyze a patient with glaucoma who had an associated pituitary macroadenoma.

ABSTRACT

Objective To investigate the effects of downstream regulatory element antagonist modulator (DREAM) on the expression of glutamate transporter-1 (GLT-1) in spinal cord in rats with bone cancer pain and morphine tolerance.Methods Sixty female healthy Sprague-Dawley (SD) rats weighing 200 ~230 g were randomly divided into tow groups,group Ⅰ cancer pain (CP,n =48) and group Ⅱ Sham (S,n =12).Cancer pain in each group was produced by inoculation of syngenetic Walker 256 rat mammary gland carcinoma cells (5 × 105) to left tibia.Pain threshold to mechanical stimulus was measured before (baseline) and after the surgical procedure.From 14 d to 18 d after the inoculation of carcinoma cells,36 rats from group CP received subcutaneous injection of morphine at 3 times per day with doses increasingly from 10 mg/kg initially to 20 mg/kg,30 mg/kg,40 mg/kg,and 60 mg/kg.Equal volume of normal saline was applied to the 12 rats left in group CP.On 19th day after the carcinoma cells inoculation once subcutaneous injection of morphine at 3mg/kg was performed in all rats in group CP.From the next day,the rats in group CP ever receiving injections of morphine for 5 days were randomized into thre subgroups,including subgroups morphine tolerance (MT,n =12),vehicle (V,n =12),and RNAi (R,n =12).They were injected intrathecally with 20 μl of normal saline (NS),10 μl vehicle plus 10 μ1 NS,and 10 μ1 of DREAM-shRNA plus l0 μ1 NS,respectively,once a day for 5 days.Focusing on the affected limb,mechanical pain threshold was measured one day before surgery (T0),and at day7 (T1),day 14 (T2),day 18 (T3),day 19 (T4),day21 d (T5),day 25 (T6),and day 28 (T7) after surgery.The animals were sacrificed at day 28 after the procedure.The lumbar 4 segments in rats were removed for detection of DREAM and GLT-1.Results The mechanical threshold was significantly decreased at T1 compared to the baseline in all groups,returned to the baseline at T2 ~ T7 in group S,at T4 in group CP,and at T2 in group MT,V,and R,but remained low at T5 ~T7 in group CP,and at T3 ~T7 in group MT,V,and R.Compared to that at T1,it was decreased at T2 ~T3 and T5 ~ T7 in group CP,at T4 ~ T7 in group MT and V,and at T4 ~ T5 in group R,going back to the baseline at T4 in group CP and at T2 in group MT,V and R,and increased at T6 ~ T7 in group R.Compared to that in group S,the mechanical threshold in group CP,MT,V and R was decreased,and lower at T2 in group CP and at T4 in group MT,V and R.Compared to that in group CP,the mechanical threshold was significantly higher at T2 ~ T3 but lower at T4 in group MT,V,and R,decreased at T5 in group R and at T5 ~ T7 in group MT and V.The mechanical threshold was increased at T6 ~ T7 in group R and higher than that in groups MT and V.The expression of DREAM,compared to that in group S,was down-regulated in other groups.Compared to group CP,increment was shown in groups MT and V,and decrease was exhibited in group R.It was cut down in group R compared to that in groups MT and V.Compared to group S,GLT-1 was decreased in other groups.It was down-regulated in groups MT,V and R compared to group CP.Conclusions DREAM is involved in the development of allodynia after morphine tolerance in rats with bone cancer pain.No evidence in this study supports a link between DREAM and GLT-l in spinal cord.

ABSTRACT

RESUMO As síndromes hiperandrogênicas englobam doenças que se manifestam através de um aumento da atividade biológica dos androgênios e podem ter origem em patologias neoplásicas ou funcionais. Os tumores ovarianos secretores de androgênios constituem cerca de 1 por cento das neoplasias do ovário. O tumor de células esteroides é um dos tipos mais raros, sendo responsáveis por menos de 0,1 por cento de todos os tumores ovarianos. São habitualmente benignos, de pequenas dimensões e unilaterais. Neste relato, apresenta-se o caso raro de um tumor unilateral de células esteroides. Paciente do sexo feminino, 60 anos, por hirsutismo, hipertorfia do clitóris e elevação dos níveis séricos de estradiol, com quatro meses de evolução. Apresentava níveis elevados de testosterona total e de 17-OH-Progesterona.


ABSTRACT Hyperandrogenic syndromes include diseases that manifest through an increased biological activity of androgens and that can originate from neoplastic or functional diseases. Androgen-secreting ovarian tumors represent about 1 percent of ovarian neoplasias. Steroid cell tumors are among the more rare types which account for less than 0.1 percent of all ovarian tumors. They are usually benign, of small dimensions and unilateral. We report here a rare case of a unilateral steroid cell tumor. A 60-year-old woman was seen after four months of evolution of hirsutism, clitoris hypertrophy and elevation of serum estradiol levels. Her total testosterone and 17-OH-progesterone levels were also increased.
